Being Unfit May Be Almost as Bad for You as Smoking


Being out of shape could be more harmful to health and longevity than most people expect, according to a new, long-term study of middle-aged men. The study finds that poor physical fitness may be second only to smoking as a risk factor for premature death.
It is not news that aerobic capacity can influence lifespan. Many past epidemiological studies have found that people with low physical fitness tend to be at high risk of premature death. Conversely, people with robust aerobic capacity are likely to have long lives.
But most of those studies followed people for about 10 to 20 years, which is a lengthy period of time for science but nowhere near most of our actual lifespans. Some of those studies also enrolled people who already were elderly or infirm, making it difficult to extrapolate the findings to younger, healthier people.
So for the new study, which was published this week in the European Journal of Preventive Cardiology, researchers from the University of Gothenburg in Sweden and other institutions turned to an impressively large and long-term database of information about Swedish men.
The data set, prosaically named the Study of Men Born in 1913, involved exactly that. In 1963, almost 1,000 healthy 50-year-old men in Gothenburg who had been born in 1913 agreed to be studied for the rest of their lives, in order to help scientists better understand lifetime risks for disease, especially heart disease.
The men completed baseline health testing in 1963, including measures of their blood pressure, weight and cholesterol, and whether they exercised and smoked. Four years later, when the volunteers were 54, some underwent more extensive testing, including an exercise stress test designed to precisely determine their maximum aerobic capacity, or VO2 max. Using the results, the scientists developed a mathematical formula that allowed them to estimate the aerobic capacity of the rest of the participants.
Aerobic capacity is an interesting measure for scientists to study, because it is affected by both genetics and lifestyle. Some portion of our VO2 max is innate; we inherit it from our parents. But much of our endurance capacity is determined by our lifestyle. Being sedentary lowers VO2 max, as does being overweight. Exercise raises it.
Among this group of middle-aged men, aerobic capacities ranged from slight to impressively high, and generally reflected the men’s self-reported exercise habits. Men who said that they seldom worked out tended to have a low VO2 max. (Because VO2 max is more objective than self-reports about exercise, the researchers focused on it.)
To determine what impact fitness might have on lifespan, the scientists grouped the men into three categories: those with low, medium or high aerobic capacity at age 54.
Then they followed the men for almost 50 years. During that time, the surviving volunteers completed follow-up health testing about once each decade. The scientists also tracked deaths among the men, based on a national registry.
Then they compared the risk of relatively early death to a variety of health parameters, particularly each man’s VO2 max, blood pressure, cholesterol profile and history of smoking. (They did not include body weight as a separate measure, because it was indirectly reflected by VO2 max.)
Not surprisingly, smoking had the greatest impact on lifespan. It substantially shortened lives.
But low aerobic capacity wasn’t far behind. The men in the group with the lowest VO2 max had a 21 percent higher risk of dying prematurely than those with middling aerobic capacity, and about a 42 percent higher risk of early death than the men who were the most fit.
Poor fitness turned out to be unhealthier even than high blood pressure or poor cholesterol profiles, the researchers found. Highly fit men with elevated blood pressure or relatively unhealthy cholesterol profiles tended to live longer than out-of-shape men with good blood pressure and cholesterol levels.
Of course, this study found links between poor fitness and shortened lifespans. It cannot prove that one caused the other, or explain how VO2 max might affect lifespan. However, the findings raise the possibility, as the scientists speculate, that by strengthening the body, better fitness may lower the risk of a variety of chronic diseases.
This study also involved men — and Swedish men at that. So whether the findings are applicable to other people, particularly women, is uncertain.
But “there is no reason not to think” that the rest of us would also share any beneficial associations between fitness and longevity, said Per Ladenvall, a researcher at the Sahlgrenska Academy at the University of Gothenburg, who led the study. Past studies involving women have found such links, he said.
Encouragingly, if you now are concerned about the state of your particular aerobic capacity, you most likely can increase it just by getting up and moving. “Even small amounts of physical activity,” Dr. Ladenvall said, “may have positive effects on fitness.”

Belly fat: What's the best way to get rid of it?

Muffin top, spare tyre, blubber, belly fat, beer belly - a multitude of names but they all mean the same thing. Abdominal fat. And in the summer many turn to quick fixes to get rid of it, writes Saleyha Ahsan.
The problem with belly - or abdominal - fat isn't just the way it looks on the beach. It could be a sign that your health is at risk.
Belly fat is around our midriff - the subcutaneous fat that we are referring to when we ask if you can pinch more than an inch and also the visceral fat around our organs, like the liver, pancreas and intestines.
Last year I had an MRI scan which showed me the fat I had around my own organs and it was a sobering moment. It can lead to adverse health effects.

Visceral fat is believed to be more metabolically active than subcutaneous fat and has a higher turnover. If you gain weight you put it on there first - and if you lose weight, you lose it from there first. Although it is considered the more dangerous to our health, the good news is that it is easier to shift than the subcutaneous fat. But the question is how.
On health and fitness websites you can see the promise of "spot reduction" of belly fat. These are the quick-fix tricks that promise to help melt fat away. But how reliable are they?
The Trust Me I'm A Doctor team have done some experiments to finally get to the bottom of it.

So how do you get rid of it? We recruited 35 volunteers along with two experts in the field of exercise and diet.
All the volunteers carried abdominal fat which gave them waist sizes that put them in the danger zone for type-2 diabetes and heart disease.
Fredrik Karpe, a professor of metabolic medicine from the University of Oxford, and Prof Dylan Thompson, from the University of Bath, each had to come up with two methods to try with the four groups, aligned with their own areas of expertise.
Before the experiment, each person had their health parameters taken. One of the most crucial measurements was a DEXA scan - short for dual X-ray absorptiometry - to get a really detailed picture of the amount of fat everyone had and where it was distributed.

Other health parameters were measured - resting heart rate, blood glucose, blood lipids, weight, blood pressure and of course waistline measurements.
Prof Thompson took on two groups for two types of exercises, while Prof Karpe took two groups for two dietary interventions.
The first of Prof Thompson's groups wore monitors to check activity throughout the day and the volunteers were told to eat as they normally would. He gave them simple lifestyle changes to make them more active and increase their daily step count.
Prof Thompson's second group was given a classic internet belly-fat fix - sit-ups. Each volunteer did six exercises three times each - every other day over the six-week period.

Lie on your back, knees bent and feet flat on the floor, hip-width apart. Place your hands on your thighs, across your chest or behind your ears. Slowly curl up towards your knees until your shoulders are about three inches off the floor. Hold the position for a few seconds and lower down slowly.

The third group, monitored by Prof Karpe, was given the task of following another popular online belly fat reduction suggestion - consuming up to three glasses of milk (one litre) a day.




There has been research suggesting that dairy consumption might promote fat excretion. Fat comes out in the faeces rather than being absorbed.
The fourth group was the diet group. They were not told to change the type of food they ate.
Their task was simply to reduce portion size, measuring foods with their hands and fingers. They were also told to cut out snacks between meals. The aim was to make this as easy to follow as possible - if people are given separate meal plans from the rest of the family it can be difficult to follow and more expensive.
A dietician supported this group through the process, giving the volunteers coping strategies if they were suffering from hunger pangs. She had individual sessions with everyone at the start and continued to support them during the six weeks with phone calls.
At the end of the six-week period, all the participants were tested again on waistline, blood cholesterol, blood glucose and blood pressure.

The results were rather revealing. The first exercise group didn't lose any fat, but their health markers vastly improved, with drops in blood pressure and one participant even reducing their blood glucose levels from being in the diabetic range to normal.
The sit-up group didn't lose any weight and didn't get any healthier through their parameters. But they did lose an impressive 2cm (0.8in) from their waistlines.
Prof Thompson explained that this was due to an improved core strength and the muscles in the abdomen just holding their fat better. A bit like a muscle version of body-shaping underwear. But a good core is beneficial for protection against lower back pain and improves posture.
Turning to Prof Karpe's groups - the milk drinkers didn't show any change in weight or health. However although they were being asked to consume an extra 400 calories a day they didn't gain any weight or fat. Prof Karpe thought this might be to do with them being too full from milk to eat as they normally do.

The clear winner was the diet-control group. This group lost a collective 35kg (77lb) between them which was an average of 3.7kg (8.2lb) each over just six weeks. Their average waistline reduced by 5cm (2in).
The DEXA scan readings were the most interesting with 5% less body fat and an impressive 14% reduction of the dangerous visceral fat inside the abdomen. This group lost overall body fat as well as abdominal fat. They also saw improvements in the health parameters.
But they did lose some muscle tone from their legs, which is not a good thing.
Thus, the conclusion is that if you want to bust the belly fat in a healthy and effective way, it's the age-old advice of a combination of diet and exercise. Leave the fads where you found them.

5 Things People Misunderstand About Mental Strength


Decades of research have gone into studying how people can grow stronger by changing the ways they think, feel, and behave. But there are a lot of articles on mental strength and mental toughness that just aren’t accurate, so I wanted to take the opportunity to clear up some of the biggest misconceptions about mental strength.

Here are five truths about mental strength:

1. Mentally strong people are OK saying they can’t do something.


Mental strength doesn’t come with super powers. There are plenty of things mentally strong people can’t do—and they’re not afraid to admit it. But, sadly, I see a lot of articles that claim, “Mentally strong people never say, ‘I can’t.’”

In reality, mentally strong people are happy to acknowledge their weaknesses. When it makes sense to work on them, they do. When a weakness is something they aren’t particularly motivated to change, they don’t worry about it.

2. Developing mental strength doesn’t mean you can’t give up.


Lots of articles claim mentally strong people never give up. But just like everyone else, mentally strong people’s goals change. When the cost of working toward something starts to outweigh the benefit, mentally strong people give up and move on. They’re strong enough to do that.

They don’t waste time working to solve a problem that doesn’t have a solution. And they aren’t so prideful that they feel they have to keep going, no matter the cost. That’s because they’re mentally strong—not just acting tough.

3. No specific diet will help you build mental strength.


There are lots of cringeworthy recommendations about what you should consume to build mental strength. But mentally strong people are just as diverse in their diets as any other group.

Some drink coffee. Some are vegetarians. Some like cereal for breakfast. So don’t believe any articles that say you have to consume certain foods if you want to build mental strength.

While there is a clear connection between maintaining a healthy body and a healthy mind, there’s more than one way to stay healthy. What mentally strong people eat for breakfast is far less important than their daily health habits. Just as there are a lot of different diets that help people grow physical muscle, there are many different ways to build mental muscle.


4. Becoming mentally strong doesn’t mean you’re always positive.


While mentally strong people often choose to look for the silver lining, they’re not unrealistically positive. In fact, being overconfident or unrealistically positive can leave you unprepared for the realities you’re about to face. Yet I see many articles that insist mentally strong people are “always positive.” And this simply isn’t true.

Building mental strength is about learning how to regulate your thoughts so they’re not overly negative or overly positive. Mentally strong people are clearheaded about the realities of life, even when those realities are hard to face.


5. Building mental strength won’t necessarily make you a high achiever.


There are some mentally strong people who have achieved incredible things. But plenty of others fall elsewhere on the achievement scale—and they’re OK with that. Be wary of any article that claims you have to be a billionaire or an elite athlete to be strong.

Mental strength is about living according to your values. For some mentally strong individuals, that means valuing time with family over earning money. And it means doing their best at whatever they do—whether it’s flipping burgers or being a stay-at-home parent.

Day Care Infections May Mean Fewer Sick Days Later


All three of my children started out in day care as infants, and the day care center was, in so many ways, at the center of our lives for years. The teachers taught us most of what we knew about young children (including, I am sorry to say, the difference between well-meaning parents and truly talented professionals). The day care cohort provided my children with their close friends (who keep turning up in their high school and college classes). We even bought our house so we could live near the day care center.

But yes, there were the infections. The worst battles my husband and I had were fought when a small child had to stay home with fever or diarrhea, and we had the eternally nasty whose-work-day-is-more-important-mine-or-yours-and-just-what-makes-you-think-so discussion. On the other hand, as our children got a little bigger, the infections essentially vanished — they had nearly perfect attendance records by the time they got to kindergarten and beyond.

In a study published last month in the journal Pediatrics, researchers in the Netherlands followed a large group of children over the first six years of their lives, looking at how often doctors diagnosed acute gastroenteritis, the stomach bugs so familiar to parents; 1,344 out of the total 2,220 children studied attended day care during the first year of life. Being in day care as an infant increased a child’s risk of having acute gastroenteritis in the first year of life, but it also had a protective effect after that.

Interestingly, the protective effect lasted at least till age 6, which is as far as the study went; the children who were in day care by the age of 1 had more gastroenteritis earlier, but the non-day care children got sick more often as they got older. By age 6, children in the two groups averaged similar numbers of total episodes.

“We think if you are infected at an early age you build up immunity against these viruses or bacteria,” said Marieke de Hoog, an epidemiologist at University Medical Center Utrecht and the senior author on the study. “There is even a possibility that the protective effect we have seen will continue when children grow up — we need more research.”

Day care attendance is known to be a risk factor for upper respiratory infections, which are much more frequent than gastroenteritis — the average child may have as many as eight upper respiratory infections a year, to maybe one bout of gastroenteritis, said Dr. Timothy Shope, an associate professor of pediatrics at Children’s Hospital of Pittsburgh, who is the co-editor of the American Academy of Pediatrics book “Managing Infectious Diseases in Childcare and Schools: A Quick Reference Guide,” the fourth edition of which is due out next month. And going to day care also puts a child at higher risk for the ear infections that can follow those upper respiratory infections.

In fact, the gastroenteritis study from the Netherlands was an offshoot of a larger study focused on respiratory infections in children, and Dr. de Hoog and her colleagues published an article in 2014 in which they demonstrated a similar pattern in upper respiratory infections and ear infections: Children who attended day care in the first year of life had more infections earlier and fewer later. For the early day care group, this led to more doctor visits overall, and more antibiotics.

Some infections can be more severe or more dangerous in babies, which is probably one reason children who get sick younger get more medical attention. And there can certainly be moments in those early years of day care when, even though children are not dangerously ill, parents can feel overwhelmed by the parade of runny noses and runny bowel movements.

There is a more complex relationship between day care attendance and the risk of developing asthma and eczema, where there are several other important factors, like family history, antibiotic exposure, and the risk of infection with one particular virus called respiratory syncytial virus, or R.S.V., which has been linked to developing asthma. Still, Dr. Shope said, day care attendance may protect against these so-called “atopic” diseases, which are related to hypersensitivity reactions in which the body’s immune system is overreactive, and it has been suggested that this connects to the “hygiene hypothesis,” that early exposures may be beneficial for the immune system.

There are three basic lines of prevention for bringing down the frequency of infections in day care children, and the most effective is immunization. We immunize against several of the most common organisms that cause gastroenteritis (oh, how I wish there had been a vaccine against rotavirus when my children were born — that diarrhea lasted for weeks) and also against some of the organisms that can complicate respiratory infections. But we have to get better at making sure all young children get the influenza vaccine, which unfortunately has to be given every year, since the virus is particularly dangerous to children.

Infection control is also important in day care, especially around diaper changing and hand-washing. However, there are limits to how much young children can cooperate regarding what they put in their mouths, or whether they cover their coughs. Studies show some benefit of infection control measures, but “not as much as we would like,” Dr. Shope said.

Then there is the always complex pediatric decision about who needs to stay home. Although there are very specific recommendations around diarrhea and diaper changing, for example, it’s also true that with many viral illnesses, children are infectious before symptoms develop and after they cease, while other children are infected and infectious but never develop symptoms, so the protective value of excluding the symptomatic children is limited.

I often see parents who are told their children have to stay home from day care, and I still identify with them. So does Dr. Shope. “When I’m seeing a typical parent with a child under a year who’s in child care, and they’re missing work, they’re wondering if something is wrong with the child,” he told me. “I tell them this is normal, they’re making an investment for the future, their child is less likely to be ill going into kindergarten when other children raised with less contact are more likely to be ill.”
